Support to start attending an educational setting, move between settings or when a young person prepares to move into adulthood.

Transition is a process or period of change. It can be a time of uncertainty, and we understand that transition can be a tricky time for our children and young people but with the right support they can develop the resilience and skills needed to be successful.

To support transitions, we have created some guidance and support pathways for:

  • Starting Nursery
  • Starting Primary School
  • Starting Secondary School
  • Moving between educational settings
  • Beyond school
