An Education, Health and Care Plan (EHC Plan) is for children and young people who need more support than their school or setting can reasonably provide after following the Graduated Approach.

An Education, Health and Care Needs Assessment (EHC Needs Assessment) can be requested from the Local Authority and if they agree, an assessment will be carried out to see if an EHC Plan is needed.
