Transition Clinical Nurse Specialist

The role of the transition nurse is to improve the experience of transition for young people and their families, by providing and co-ordinating holistic care to young people and their families, to facilitate the transition process over to adult services.

The transition nurse specialist role plays an important part in liaising and informing health and social care staff to ensure patients receive the appropriate care for their needs and are involved and informed throughout the journey. 

Support

  • Provide a point of contact for professionals who oversee young people approaching transition, which starts at the age of 14.
  • Provides expertise in relation to transition, leading the transition service for young people with complex or chronic long term, or life limited conditions. 
  • Developing partnerships with young people, their families and professionals from paediatric to adult services. 
  • Support all services to help and ensure transition programmes and pathways to adult services are embedded.
